COMPARATIVE ASSESSMENT OF THE PROGNOSTIC SIGNIFICANCE OF VARIOUS MARKERS OF PREMATURE BIRTH

The study presents the results of a randomized cohort study conducted on 156 pregnant women with the risk of preterm birth, aimed at determining the prognostic value of fetal fibronectin, placental alpha-microglobulin-1, and insulin-like growth factor-binding protein-1 in the cervix and vagina. The methods of the study were applied to pregnant women with a cervical length of 10-25 mm based on ultrasound cervicometry. Comparative analysis of the obtained parameters allowed to conclude that placental alpha-microglobulin-1 and insulin-like growth factor binding protein-1 are more informative (90.9% and 84.1%) in diagnostics of deep premature labor (22-28 weeks) and have high prognostic value, whereas fetal fibronectin allows to predict labor at this term only in 50% of cases and its prognostic value is low. At 34-37 weeks, fetal fibronectin is the most informative marker in diagnostics of premature labor and in most cases is positive. According to the authors, all these markers are characterized by high sensitivity and specificity at different stages of pregnancy, which is important for application in clinical practice.
