EFFECT OF HETEROTOPIC OSSIFICATION ON HIP JOINT RANGE OF MOTION AND CLINICAL-FUNCTIONAL PARAMETERS AFTER TOTAL ARTHROPLASTY

Abstract

The research article provides information about a study conducted to investigate the frequency rate and impact of periarticular heterotopic ossification (HO) events on the range of motion and clinicalfunctional outcomes following total arthroplasty in patients with dysplastic coxarthrosis. The study included 135 patients aged 28-79 (mean age: 50.9±0.9 years) who applied to the arthroscopy and arthroplasty (125 women, 10 men) was conducted based on the results of surgical treatment. According to the age group, there were 114 patients under 60 years (84.4%), and 21 patients over 60 years (15.5%). Total arthroplasty was performed in all patients. Joint range of motion indicators were assessed using a goniometer, range of motion was assessed using the Merle d'Aubigne Postel method, clinical complaints were assessed using the WOMAC osteoarthritis index, functionality was assessed using the Harris Hip Scale (HHS), and pain localization and severity were assessed using the Visual Analogue Scale (VAS). The study showed that articular HO occurred in 17 patients (12.6%) after total arthroplasty. After the surgery, 97 patients (71.9%) had pain in the hip-pelvic joint area, 37 patients (27.4%) had mild pain, and 1 patient (0.7%) had moderate pain. In the analysis of gait parameters, complete loss of walking in 7 patients (5.2%) and severe uneven gait in 128 patients (94.8%) were determined before surgery, but after total arthroplasty all patients recovered, of which 106 patients (78.5%) no uneven gait was recorded. Only 29 patients (21.5%) had mild (weak) uneven gait. During the average observation period of 18 months, the clinical and functional indicators of the joint improved in all patients. After the surgery, clinical complaints decreased from 65 points to 10 points according to the WOMAC osteoarthritis index, functional indicators of the joint increased from 10 points to 75 points according to the Harris Hip Scale, the results of the Merle d'Aubigne Postel scale results increased from 8 points to 16 points.
