The article discusses the relevance of blepharoptosis. Separately, the reasons leading to complications after surgical correction of blepharoptosis, especially the recurrence of the disease and the development of postoperative lagophthalmos, are considered. The purpose of research is to develop a technique with a decrease in the static resistance of the tarsal plate by transverse dissection and to determine the effectiveness of surgical correction of blepharoptosis after transverse dissection of the tarsal plate in the main group compared to the control group without its dissection. 155 patients (197 eyes) operated on for the period from 2014-2022 was carried out. In the control group without preliminary dissection of the tarsal plate, the percentage of poor results is significant and in relation to the operated eyes was 66.7%, while the percentage of good results was only 27.3%. In the main group, preliminary dissection of the tarsal plate according to our proposed method gave a weak result in 7.6% (10 eyes), a good result was observed in 91.6% of the operated eyes (120 eyes).
