Abstract

Article presents the results of a study carried out to substantiate the implementation of simultaneous surgical treatment for hemorrhoids and anal fissure using transanal hemorrhoidal dearterialization. The study involved 177 patients. Group I - 60 patients who underwent MiliganMorgan hemorrhoidectomy and anal fissure removal, II - 60 patients who only had anal fissure removed, III - 57 patients operated with original method. laser Doppler flowmetry was performed in groups II and III to assess the blood flow intensity in the operated area. Research has shown that simultaneous excision of anal fissure and transanal hemorrhoidal dearterialization can improve the results of treatment for patients with a combination of hemorrhoids and anal fissures by simultaneously reducing the blood supply to the cavernous tissue of hemorrhoids and improving blood flow in the area of previous existence of the fissure by eliminating the defect in the mucous membrane of the anal canal and sphincterospasm with minimal trauma to the anal canal.
