The article provides information about a study conducted to compare the outcomes of laparoscopic and laparotomic surgical treatments for endometrial cancer. A retrospective analysis was conducted on 1532 patients diagnosed with endometrial cancer who underwent surgical treatment at the Oncology Clinic of Azerbaijan Medical University between 2018 and 2025. The distribution of cancer stages among patients was as follows: 43.2% IA, 18.7% IB, and 20.5% stage II. Statistical analysis confirmed significantly better survival rates in the laparotomy group compared to the laparoscopic group (p = 0.010 and p = 0.007). The recurrence-free period was approximately 2 years in the laparoscopy group and 3.5 years in the laparotomy group. The study observed that the laparotomic method demonstrated superior efficacy and reliability for non-endometrioid histological subtypes of endometrial carcinoma. A significant correlation was identified between the surgical approach and patient prognosis. These findings suggest that laparotomy may offer improved oncological outcomes, particularly in cases with non-endometrioid histology, warranting careful consideration of surgical strategy in clinical practice.
