The article presents a scientific study conducted to investigate the correlation between changes in the leukocyte composition of peripheral blood and the clinical forms of acute calculous cholecystitis (ACC). A total of 116 patients were included in the study. Patients were divided into three groups: Group I included 53 patients with the catarrhal form of inflammation, Group II consisted of 38 patients with phlegmonous cholecystitis, and Group III comprised 25 patients with the gangrenous form of inflammation. Dynamic studies conducted before and after cholecystectomy in these 116 patients demonstrated that leukocyte levels correlate with the clinical form of ACC. Increasing inflammation severity and the progression of destructive changes in the gallbladder were accompanied by elevated leukocyte counts and alterations in leukocyte differential parameters. Based on the results of this study, it was concluded that leukocyte differential counts can serve as a more sensitive biomarker for the reliable early diagnosis of destructive forms of ACC and for assessing postoperative outcomes.
