The article discusses the causes of impaired occlusal function of the upper eyelid and the development of postoperative lagophthalmos during surgical correction of blepharoptosis. The aim of the study was to determine the factors influencing the development of postoperative lagophthalmos during surgical correction of blepharoptosis and, on this basis, to create a technique that maximally prevents the development of lagophthalmos and preserves the occlusal function of the upper eyelid. A total of 131 eyes were operated on from 2015 to 2023. In 98.5% of cases, we obtained a good result with complete preservation of the occlusal function of the upper eyelid. In 2 cases (1,5%), persistent lagophthalmos of 2 mm occurred, which did not cause inconvenience to the patient. Thus, the effectiveness of this technique for maximum preservation of the occlusal function of the eyelid after surgical correction of blepharoptosis has been proven and this technique can be recommended for practical use.
